If your beneficiary’s Medicare card was lost, stolen, or destroyed, you can request a replacement online using your personal my Social Security account to access Representative Payee Services.

From there, select the “Medicare Replacement Card” link in the navigation bar on the right side of the page to make your request. If your mailing address on the screen is not correct, be sure to update it before ordering the replacement Medicare card.

The Medicare card will arrive in the mail in about 30 days at the representative payee’s address on file with us. You can only request one replacement card per beneficiary in a 30-day period.

The Medicare card is proof of a person’s Medicare health insurance. If you need immediate proof of Medicare coverage for the beneficiary you represent, you can print a benefit verification letter for the beneficiary. You can use your personal my Social Security account to access Representative Payee Services.
