If you lost your card, you may not need a replacement. In most cases, simply knowing your Social Security number (SSN) is enough. But, if you do need a replacement, you can complete your application online or in-person.

Visit our SSN and Card webpage and answer a few questions to find out the best way to apply for your replacement card. In most states, you can use your personal my Social Security account to request a replacement card. If you are not able to do that, you can still begin your application online and self-schedule an appointment to complete it in your local Social Security office or Card Center. Scheduling online can save you time when you come to visit us to complete the application.

If you cannot complete the application online using my Social Security, you will need to provide the required documents in person. You are required to provide different documents depending on your citizenship and the type of card you are requesting. If you do not begin the application online, you will need to complete a paper Application for a Social Security Card (Form SS-5) and bring your required documents to a local Social Security office or Card Center. To save time, you can download, complete, and print the paper application SS-5 form before you arrive.

There is no charge for a replacement card.
